Perhaps the uninstall step was important, I'm not sure, but you could try that (and also 10.1.4 may have fixed some more issues). I have a Decklink 4K Extreme in a Sonnet PCIe Thunderbolt 2 chassis. You also get reference input, analog audio in, RS-422 PTZ control, and Ethernet for connecting hardware or software control panels All video inputs feature. I just upgraded to Desktop Video 10.1.4 from being on 9.9.3 for a while, and all went well except for a known issue (10.x versions have a problem with capturing 1080p60 video, which only a few cards of their support).
